Job Summary
Reports to the Manager, Cardiac Rehabilitation (CR), Heart Health Program. As a member of the CR team, participates in developing, implementing and evaluating the Cardiac Rehabilitation Services within the Program in accordance with the Vision and Mission of the Capital Health Region and within the Purpose, Mission and Objectives of the CR Program.Responsible for providing skilled professional services to people at risk, people with known heart disease, their partners and families, and the community. Develops standards and processes, and monitors and evaluates services for the CR Program. Acts as a clinical resource for physicians, other health care professionals, and the community in matters relating to prevention of heart disease and cardiac rehabilitation. Provides in-services and information to hospital staff and physician groups. Provides assessment, support and information.In collaboration with members of the CR Team provides emotional support, information and education to people and families at the clinic as well as liaises with hospital and community program partners to ensure continuity of care and to achieve optimal clinical outcomes and patient satisfaction.

Qualifications
Education, Training and Experience
Baccalaureate Degree from a recognized university Nursing program or related health field or evidence of an equivalent combination of education, training and experience.
Current practicing membership with the College of Registered Nurses of British Columbia (CRNBC) required.
A minimum of three years recent related experience including experience in cardiac rehabilitation, cardiac care, ECG interpretation, patient education, behavioural counselling, and interpretation of exercise stress testing and other related activities.
Demonstrates knowledge in the current theories, technologies and practices in cardiac disease prevention, rehabilitation and risk factor reduction.
Knowledge of Adult Education learning principles.
Advanced Cardiac Life Support (ACLS) certification.
American College Sports Medicine exercise specialist certification preferred.

Demonstrated Skills and Abilities
Ability to deal effectively with people who have heart disease or who are at risk for heart disease and their families, physicians, other members of the CR team, hospital staff, and community agencies.
Effective interpersonal, organizational and teaching skills.
Ability to work independently and manage a varied workload.
Knowledge of cardiac related diseases and cardiac disease prevention and rehabilitation, related diagnostic tests and procedures and their effect on patients, and their families.
Ability to assess patient's needs and respond effectively.
Knowledge of the resources and support services required and available to patients and their families.
Proficient in providing effective adult education to a variety of professional and lay learners.
Experience leading and participating in quantitative and qualitative research projects.
Leadership ability and ability to lead change.
Physical ability to carry out the duties of the position.
Ability to communicate effectively both verbally and in writing.
